1. GENERAL PROVISIONS

These Privacy Terms regulate the processing of personal data collected through the website www.amselclinic.eu, to which every person who fills out the online form must agree.

The privacy conditions of AS Bariatric Services are based on the General Data Protection Regulation of the European Union (GDPR – General Data Protection Regulation) and the Personal Data Protection Act of the Republic of Estonia and other legislation that regulates the service provider’s field.

2. PURPOSE AND LEGAL BASIS OF DATA PROCESSING

The legal basis for data processing is the consent given on the online form by the person filling out the online form (data subject).

Collected personal data is processed only for the purpose of providing the Service, which is:

assess and give a preliminary answer as to whether bariatric surgery is indicated for the person making the request, whether he is a suitable candidate for surgery and which surgery we recommend.
contacting the person using the contact information provided by him.
sending additional information to the person’s e-mail about the clinic and the service. At any moment, the person has the opportunity to opt out of e-mails containing additional information by removing himself from the corresponding e-mail recipient list by clicking on the corresponding link in the received e-mail.

3. WHAT DATA IS COLLECTED

We only collect and use data that is necessary to achieve the above purposes.

Bariatric Services mainly processes the following personal data: first and last name, phone number, e-mail address, height, weight, age.

In case of surgery, also: residential address, social security number/date of birth, name and phone number of an emergency contact person.

In addition, we collect special (sensitive) personal data, which is data about your state of health

All the mentioned data is submitted by the person to us through our online form voluntarily. Further data processing depends on whether the person is eligible to receive medical services. It is then necessary to fill out an additional comprehensive form before providing the service.

4. DATA STORAGE

Personal data collected for the provision of the service described in point 2 will be stored for up to 1 year from the last contact. Personal data that is collected and used for the provision of healthcare services is stored in accordance with the provisions of the Act on the Organization of Healthcare Services.

5. RIGHTS OF THE DATA SUBJECT

A person has the right at any time:

consult your Personal Data;
the right to submit objections and information about the right to transfer personal data;
the right to file a complaint with the supervisory authority;
consent to withdraw personal data processing;
request correction of personal data, restriction of processing, termination of processing, deletion of collected Personal Data (unless the legislation provides otherwise)
To this end, the Person submits a corresponding request to the e-mail address info@amselclinic.eu. The request for personal identification must come from the same e-mail address from which the consent for data processing has been given. Withdrawal of consent is not retroactive.

6. TRANSFER OF PERSONAL DATA TO THIRD PARTIES

All personal data of the customer that has become known to us is treated as confidential information. We will not transfer your data to any third party, unless it is necessary for the provision of our services or if required by law.

7. SECURITY

Bariatric Services AS collects and stores personal data in electronic form.

When filling out online forms, the data flow between the website visitor’s device and our website is encrypted.

Sending a copy of the completed online form to the e-mail address of the person filling in (if requested by the person filling out the online form) is done by e-mail.

However, data transmission over the Internet or email is never 100% secure. Consequently, we cannot guarantee 100% the security of any information transmission.

When we receive your transmitted data, we will do everything we can to ensure their security in our system. Your data is stored on secure servers and protected by the use of a firewall and password-required access.
